PLEASE HELP My generals keeps getting a suspicous death.

So I searched the forums but didn't saw this before or Iv' might overlooked it.
I am playing with Britain but my generals in Spain and France keep getting a suspicious death. Is this a bug or flaw in AI? I don't see any hostile spies. on the map. If it are spies then shouldn't I be able to see them if they act?

Anyone had this problem too and how did you fixed it?

Update: now its happening to my agents (gentleman) too!!

SO is this a spy? But then why can't I see him when he acts?

    In NTW a spy is only revealed if it fails an action or if another spy or agent finds it. The spy has a less likely chance of detection if:

    Not on a road
    Not near any other agents

    That's all I know
