In my campaign as Beastmen I've repeatedly gotten the wrong character avatar/card and name pop up for mine and other faction characters. The issue seems to only affect the pop-up you get after hero-activity/sometimes battle outcomes like when a lord dies in battle. Worth noting is that this bug started happening about 150-200 turns in, or it was atleast at that time that I started taking note of it.

Example for bugtesting
My Gorebull "Aabrqu Death-Maker" is being reported in world events as the Bloodhandz Warboss "Scunk Bootlicka" as can be seen in image 1. I also added in another picture 40 turns later where the same herocard is shown for Aabrqu Death-Maker (That he is Scunk Bootlicka) and is showing him as a level 1 Orc in the activity log, while he is infact a Gorebull and level 3 (Image 2).

How to replicate the example
Step by step: Load up save file --> Use the Gorebull "Aabrqu Death-Maker" --> Do a hero activity (I used Assassinate in the picture) --> Get the result that it was, in fact, Scunk Bootlicka that made the hero activity.

I've attached the files that were asked for. However I've noticed my modified.log is 0B, if I've done some mistake or it's the wrong file just let me know and I'll try to find the correct file, if it's the wrong one.

I'd also like to say that this bug has been very amusing, having Marienburg Brayherds, dwarven lords shown as Beastmen etc. But I hope this information and savefile will help with solving what the issue could be.

    I played a few more turns to see if I could get something that looked like the Marienburg brayherd in the reddit post and I managed to get my next brayherd with a weird faction.

    A few turns in I killed an enemy to get a pop up. It resulted in the enemy killed not even showing up, looking like this:

    The next turn I managed to get a brayherd spawn and it shows as this:

    I've added a savefile to the turn 424, if you load that up and hover over the brayherd spawn you should get the elf faction being the leader. I've not done anything to my modified log so I'll not add it in this time.
