If you are playing the greenskins (or norsca in my experience) when you defeat an enemy faction leader in battle, if you kill the faction leader in a manual battle, the confederation will not prompt.
If you autoresolve and kill the faction leader the dilema to confederate will prompt. This behavior is not consistent and should be normalized. Either killing them does not allow confederations or it does.
In my save file, I defeated and killed Gorfang rotgut, the leader of Red fangs in battle over the end turn. No option to confederate was offered. I then seiged a settlement right after.
To my understanding If i did not actually kill the lord, he would have offered to confederate. If i autoresolved and he died they would have offered to confederate.
version 1.9.2 (DA NUTZ ‘N’ BOLTZ UPDATE) Only mod active is one for skilling lords automatically, I havn't even used it yet, should have no impact on opening the file.
I had the same thing happen to me during the end turn with a Norsca campaign. I thought the bug was related to winning the battle during an end turn sequence since I received the confederation event when I beat him during my turn. But then again I did autoresolve the 2nd battle so I think you are right about the manual battle piece being the problem.
Confederation for Greenskins was working perfectly fine for me until today's update to 1.9.2 beta patch. I started a brand new campaign on the Nuts & Bolts update to the Beta as Grimgor. I defeated Rotgut the same exact way I have 10's of times and this time I got no confed popup.
The issue is with auto-resolving. Winning with auto-resolve gives the option to confederate, winning a played-out battle doesn't give you the option to confederate.

If there is no fix today could they at least rollback the beta version so something is playable? Right now my choice is between the live version where half of melee is broken because of WS or this current beta where LL traits don't work, saves can break, and confed doesn't work.

I have found something different in my campaign
i have found that if the lord is camped inside a settlement and then defeated it doesn't proc the confederation option after battle.
I beat grimgor who was in a settlement without auto resolve and made sure to kill him also and didn't get the confed.
I tested this and beat wurzag out in the field in the middle of the bad lands and made sure to leave him alive and it then it gave me the confederation option.
In my campaign... to be fair I have not attacked a faction leader in the open... I have been attacked by a faction leader (this did successfully proc the confederation pop-up before 1.9.2 when I beat the faction leader that attacked ME)... and have attacked a faction leader inside a settlement. Both situations did not proc the confed screen post 1.9.2
